I like the idea of virtual online doctor :) Very interesting!
It reminds me the intelligent agent. It is an online agent that kids could talk to. I will do some serching and let you know :) Here is an example : http://www.agentland.com/
If you go into this website, you could talk to the agent Cybelle (on your lefthand side). For example, you could ask her "what is google? " "who are you?" "how old are you?" I've seen an intelligent agent who could interact with kids very well and answer their questions.
